Feature Idea - Legal Waiver/forms

I needed an online form that was a waiver for horse riding.  Users need to initial statements, electronically sign, agree to terms and submit.  I used a single line text field on the left with an html field to show the waiver statement to the right.  It would be nice if the initial box was closer to the html text "label" instead of the single line text being 50% of the width of the form.
