Conditional Emails/Fields - Add button

Hi.

My request is to simply add another Add button on the Conditional Emails and Fields pages of the Form Maker plugin.

Right now, the Add button for adding a new condition is placed only at the top of the page. When adding quite some conditions, this page is getting pretty long, and for adding another condition, one must scroll all the way back up to do so.

It would be much more convinient and useful to have this button also at the bottom of the page. Even only after adding some conditions, and not when the page is empty.
