feature request: Form Maker plugin

I am currently developing wordpress web app that collect user entries and users should be able to see the entries in many different designed pdf documents for download. So I was hoping that I can design PDF visually or upload a predesigned pdf file with in place fields that mapped to form fields 

for example if the pdf contain name I can write in the pdf like this

Name: {{name}}

  Actually we have Form maker PDF integration add-on that does the job, if you haven't tried it previously please give it a try and see how it works for you.
